Tuberculous meningitis is one of the serious disorders with high mortality, especially in infants and aged persons. Moreover, it occasionally may cause post-meningitis deafness even though they have fortunately recovered from the fatal illness.

Two cases are reported here, one a 64-year-old male, the other a female infant one year and 10 months old, who died of tuberculous meningitis. The temporal bones of these patients were studied histologically to determine the pathological changes in the inner ear. Both patients suffered meningo-genie tuberculous labyrinthitis bilaterally. The primary routes of transmission of the inflammatory process form the meningeal space appeared to be through the internal auditory canal by way of perivascular spaces of the modiolus. As in suppurative labyrinthitis, perilymphatic involvement occurred primarily with more marked changes in the basal turn near the round window.

Considering the marked pathological cellular changes in the inner apparatus, these cases would have resulted in a profound sensoneural hearing loss and vestibular disfunction, and in the case of the infant enough to result in a deaf-mutism.
