肺癌の側頭骨転移による顔面神経麻痺と突発性感音難聴—側頭骨病理組織所見の報告

Facial Nerve Paralysis and Sudden Sensorineural Hearing Loss Caused by Temporal Bone Metastasis of Lung Cancer

I．はじめに

　側頭骨への悪性腫瘍の転移は原発性側頭骨腫瘍とともに比較的稀である。しかし側頭骨病理の検索では必ずしも従来考えられているほど稀ではないといわれる^1）。今回われわれは肺癌の経過中末梢性顔面神経麻痺と突発性感音性難聴をきたした症例の側頭骨を病理組織学的に検索し，臨床所見との関連について検討したので報告する。

Histopathological findings of the temporal bone in a case of metastatic lung cancer developed left peripheral facial nerve paralysis and sudden sensorineural hearing loss are reported, and the relation to clinical signs and symptoms were discussed.

The tumor invaded the temporal bone via two routes: hematogenous metastasis and invasion from diffuse metastatic leptomeningeal carcinomatosis (DMLC), and the direct invasion of the seventh and eighth cranial nerves via the latter route in the internal auditory canal seemed to produce clinical symptoms. Further, in this case, tumor cells were diminished markedly in the metastatic lesion of the temporal bone, and it was regarded interesting as showing the effects of combination chemotherapy with COMP for the metastatic lesion of small cell pulmonary carcinoma.
