放線状菌の侵襲をみた乳頭状扁桃症例

PAPILLARY TONSIL INVADED BY ACTINOMYGOSIS, A CASE REPORT

　I．緒言

　口蓋扁桃（以下扁桃と略）の奇型に副扁桃ならびに形態異常としての振子様扁桃と乳頭状扁桃がある。

　振子様扁桃はさほど稀でなく，症例報告も多いが，乳頭状扁桃は稀であり，それがさらに放線状菌の侵襲を受けた症例を経験したのでここに報告するとともに，渉猟し得た文献を参考に，本症の病理組織ならびに病因について考察をこころみた。

　The pedunculated and papillary tonsils are both morphological anomalies of the palatine tonsils. The latter is so rare that only 27 cases were reported in the literatures of this country up to 1975, according to Nagai et al.

　We operated upon this rare case of papillary tonsil in July, 1975, which was pathologically found to be invaded by actinomycosis. A detailed microscopic survey of the removed tissues showed chronic inflammatory changes somewhat similar to those of chronic tonsillitis rather than those of tumors such as a papilloma.

　Therefor we arrived at a conclusion that some unknown congenital constitutional factor had probably superimposed with that of chronic inflammatory changes, including the invasion of actinomycosis, which provided for the formation of papillary degeneration.
