糖尿病に合併したRhinocerebral mucormycosisの1例

DIABETES COMPLICATED BY RHINOCEREBRAL MUCORMYCOSIS

　I．はじめに

　ヒトにおけるPhycomycetes（藻菌類）感染の最初の記載は1847年のSluyterのpulmonarymucormycosisの報告を以て嚆矢とするが，1885年Paltaufは中枢神経障害を伴つたdisseminatedmucormycosisの症例を報告した。1943年Gregoryら^1）は，①糖尿病，②片側性内および外眼筋麻痺，⑤眼瞼下垂，④脳膜髄膜炎，③急激な死の転帰，という臨床的特徴を有する3症例を報告した。本邦においてはmucormycosisの報告は少なく，耳鼻咽喉科領域における報告は，関谷^2），石倉^3），岩本^4），佐々木^5）らによる5例をみるにすぎない。われわれは最近，糖尿病に合併したrhinocerebral mucormycosisの1例を経験したので文献的考察をも併わせて報告する。

　A case of diabetes complicated by rhinocerebral mucormycosis in a man, aged 30, is reported. The patient affected with diabetes developed an acute paranasal sinusitis and the infection gradually extended into the orbital fossa and into the cerebral tissues.

　The diagnosis of paranasal sinusitis was made and polysinusectomy was performed. Laboratoryexamination of the removed sinus specimens showed the presence of mucormycosis.

　The patient was treated with intravenous administration of amphotericin B which was life-saving but sight of the right eye was lost with a continued paralysis of the ocular muscles.
