耳性脳膿瘍の1例

AN OTOLOGIC CEREBRAL ABCESS

　I．はじめに

　近年の抗生物質の進歩により耳性頭蓋内合併症は激減したが，決して絶滅したわけではなく，高橋，桜井^3）の報告によると1961年より1970年までの10年間の本邦における耳性頭蓋内合併症の報告は141例に達している。しかも一たび合併症を起こせば，その予後はなお，楽観をゆるされない。われわれは最近，きわめて重篤な状態で入院し，中耳根治手術および2回にわたる開頭手術の結果，奇跡的に一命をとりとめたが，術後に，高度の痴呆，および失語症，右片麻痺（不全型）を来たした症例を経験したので報告するとともに，このような頭蓋内合併症の治療は，内科，脳神経外科，耳鼻科が互いに協力しあつて行なわれることがきわめて重要であることを強調したい。

　A case of a complicated meningitis of aural origin is reported. The patient was operated upon twice in succession, for mastoidectomy and a cranial opening for removal of a pus pocket. After these operations the patient developed an increased cerebral pressure that required a procedure for ventriculoatrial shunt.

　These procedures enabled to save the patient's life but he was affected with a loss of memory, aphasia and paralysis of the right side of his body.
