類白血病性反応を伴つた汎発性骨髄転移および両側乳腺転移を来たした篩骨洞癌の剖検例

ETHMOIDAL CANCER IN METASTASIZED INTO THE BONE MARROW MAMMARY GLAND

　Ⅰ．はじめに

　原発性篩骨洞癌は比較的稀な疾患で本邦における頻度について，土井^1）は副鼻腔癌162例中1例（0.62％），笹木^2）は218例中2例（0.92％），小林^3）は385例中13例（3.4％），と報告している。一方，田坂^4）は大正14年11月より昭和36年1月までの本邦文献上に26例の報告があつたとしている。われわれは最近，原発性篩骨洞癌において，両側乳腺転移および類白血病性反応を伴つた汎発性骨髄転移を来たした症例を経験したので若干の考察を加えて報告する。

　A woman, aged 35, complained of swelling of the left neck with continuous low-grade fever. Biopsy of intranasal tissues revealed the presence of adenocarcinoma. With the diagnosis of the cancer of the ethmoids a radical operation was performed for removal of the ethmoids for a complete extirpation of the malignant tissues. Simultaneously with this procedure a metastatic tumor of the mammary gland that proved to be a lot ular carcinoma was discovered. A simple mastectomy was performed.

　However, the patient's blood soon began to show deterioration with immature monocytes and megaloblast with marked symptoms of leukemia. The patient's symptoms gradually worsened and died.
