Ⅰ.はじめに
欧米には多いが本邦ではきわめて稀だとされているZenker憩室(咽頭食道憩室)で,しかも文献上最年少と思われる症例を経験したので報告する。
Zenker's diverticulum is not an uncommon dsiease in foreign countries, yet, in this country it is regarded as to be extremely rare in occurrence. So far only 16 cases are reported in the Japanese literature.
The authors report a case of Zenker's diverticula in a man, aged 27, the youngest in age reported, who has been suffering of diverticulitis for 4 years.
Esophageal diverticulae are classified into those of pulsion and traction; Zenker's diverticulum belongs to the former. Although the mechanism has not been clearly explained Zenker's diverticulum is the result of protrusion through anatomically fragile portions of esophagopharyngeal junction under increased pressure. The rate of frequency is said to be 3.8% in all esophageal diverticulae. This affection appears to occur most frequently among men whose ages average over 50.
The most common complications are acute and chronic diverticulitis. Sixteen cases of diverticula cancers are reported in the U. S. A.
The treatment is surgical.
