心因性失声症の臨床的研究

A STUDY ON THE FUNCTIONAL APHONY

　Ⅰ．緒言

　心因性失声症（ヒステリー性失声症）については幾多の報告^{4）25）40）}があり，決して珍しいものではない。しかしこの疾患は耳鼻科領域では特殊な分野であり，精神科領域ではごくありふれた分野の一つであるためか，その診断の進め方は別としても，治療法となると，その記載は総論的である。特にわれわれ耳鼻科医は，精神医学的な知識を要求されるなどの理由から敬遠しがちである。私はこの4年間に10例の症例を経験し，これらから，その特徴点，また発症機構の主因など，今後の解明に貴重と思われるいくつかの手がかりを得た。同時に，これらの症例の具体的な経過の記載も今後の臨床に役立つものと考え，代表例については詳細に報告し併せて考案を加えた。

　Ten cases of the socalled, 'functional or hysterical', aphony that prevailed for the period of more than 4 years are reported.

　Some cases are reported in detail by using photographs of the affected larynx, before and after treatments, and also with the use of sound spectography from phonetical point of view. General tendency in the course of the disease is discussed for all cases.

　Through these facts, the author believes, that the cause of such a disorder is due to inhibitory effect that involves the central nervous system, especially the region around the hypothalamus, which is concerned with the center of anger, and the cerebellar cortex.

　In some cases a good treatment results were obtained by administration of specially arranged general anesthesia with 3% fluothane added for the period of 30 to 45 minutes. The details of this method is fully discussed.
