喉頭癌手術後早期死亡要因の検討

CONSIDERATIONS IN THE CAUSES EARLY DEATH OF PATIENTS FOLLOWING OPERATIVE TREATMENT OF LARYNGEAL CANCERS

　I．はじめに

　喉頭癌の治療成績は，手術，放射線療法とも他の頭頸部癌にくらべて一般に良好である。しかもStage Ⅰのような早期癌に対しては，機能を保存する喉頭部分切除術あるいは放射線療法のいずれによつても根治手術とほぼ同程度の治療効果が期待できるが，Stage Ⅲ，Ⅳといつた進行例に対しては，やはり保存手術や放射線では不確実で，どうしても確実な根治的な喉頭全摘術に委ねられるのが現状である。しかもこれら進行例に対しては，喉頭全摘術に加えて当然根治的な頸部廓清術も併用されることが多いので，全身状態の不良と相俟つて，それだけ手術侵襲の過大となり，ひいては手術死とか，術後早期死亡といつた不幸な転帰をとることもまれに経験する。

　喉頭癌手術後の早期死亡例についての検討は，喉頭癌の外科的治療成績向上への重要なる示唆を与えるものと考えるので，以下術後早期死亡要因について観察検討を加えたいと思う。

　The cause of early postoperative death of the patient following surgical removal of the cancer involving the laryngeal region may be numerous which may be relegated to the patient's pre-operative general physical condition, the rate in which the disease may progress, the method of operation adopted, surgical technic and posto-perative complications. Furthermore, the extensiveness of the surgical maneuver as well as technic to a patient advanced in age may be an added important cause.
