眩暈を伴わない反復性聴力障害の1例

A CASE OF RECURRENT DEAFNESS WITHOUT VERTIGO

Ⅰ．緒言

　内耳は聴器と平衡器とから成ることは周知のごとくである。両者はその機能こそ異なるが，その発生および構造はきわめて密接な関連を持つている。したがつて，ひとたび内耳にある種の侵襲が加わつた場合，その病変は広く聴器ならびに平衡器を障害することが考えられる。事実，メニエール病など多くの内耳疾患で聴，平衡器障害が併発している。

　しかし，きわめてまれであるが，内耳性疾患と思われる病変が聴器のみを特異的に障害し，平衡器にはみるべき症状を起こさない事態が起こることがある。われわれは最近，めまいなどの平衡器障害の症状を伴わず，可逆性の難聴を繰り返している1症例に遭遇したでの若干の考察を加えて報告する。

　A 31 years old male had a bilateral sudden deafness ten years ago. Since then his leftear had severe perceptive hearing loss and the right ear showed recurrent perceptive deafness up to 40-50 dB in the low frequencies with complete recovery, but accompaning no complaint of vertigo. The onsets of the attacks were sudden and their durations may be from several days to few months.

　The possible mechanisms of the fluctuation of hearing and the causes of the lack of vertigo are discussed.
