　Ⅰ．緒言

　近年，交通災害の激増と共に頭部外傷の症例もしばしば認められるようになり，その後遺症がきわめて大きな社会問題に発展しつつある。わが耳科学領域においてもその一つに外傷性難聴がある。

　一般に頭部外傷後の難聴は多くの場合，感音性難聴と見なされていたが，最近試験的鼓室開放術の適応拡大によりその病態に中耳伝音性障害によることが少なくない事実が明らかにされてきている。

　Four cases of posttraumatic conductive deafness were described.

　Exploratory tympanotomy revealed the interruption of the incudostapedial joint in every cases.

　Reconstruction of the separated ossicular chain was performed and remarkable hearing gain was attained.

　The mechanism of conductive deafness caused by head injury was considered and several methods of the reconstruction of the ossicular chain defect were discussed.
