耳性および鼻性髄膜炎の治療経験—とくにその髄液排除について

AN EXPERIENCE IN THE TREATMENT OF MENINGITIS ORIGINATING FROM THE EAR OR NOSE

　はじめに

　近年化学療法の発達にともなつて耳鼻咽喉科領域における頭蓋内合併症は次第に減少の傾向にあるが今日なお重篤な疾患の一つであり，その治療において適切さを欠く時は不幸な転帰をとることもあるので十分に留意してその治療にあたる必要がある。われわれは第1表のごとく最近11年間に17例の耳性頭蓋内合併症（内5名死亡）および5例の鼻性髄膜炎（内1名死亡）を経験したが，それらの症例の治療に際し髄液排除という点でとくに示唆に富んだ数例をとりあげ，ここに報告し諸家の御批判を仰げればさいわいである。

　The authors selected 7 cases of menigitis for presentation from among 21 similar cases that came within their realm of care in the last 11 years that were considered as to be caused by invasion through oto-and rhinologic origin. The treatment consisted primarily of eradicating what was considered as the disease origin along with the use of antibiotics after a proper selectivity test had been performed. A proper cerebrospinal fluid pressure was maintained by repeated spinal tapping. The authors emphasize the latter procedure as the highly important in the treatment of this disease.
