脳ヘルニヤ及び脳脊髄液漏を伴つた術後性鼻性髄膜炎の1治験例

　I．緒言

　近時化学療法の著しい進歩によつて，昔日の所謂難病で今日其の席を去つたものは非常に多い。我耳鼻科領域に於ても，難治中の難治と謂われて来た鼻性髄膜炎でさえ多くの治験例が報告されている。私の臨床に於ても過去四年間に於ける治験例は2,3に止まらない。所が最近，脳ヘルニヤと脳脊髄液漏を伴つた術後性慢性鼻性髄膜炎に遭過し，化学療法の助力を得て手術的にこれを治癒し得たので報告する。

Cerebral herniation resulted in a patient in whom repeated paranasal sinus operations were performed. The former condition was the direct outcome from the removal of the right ethmoids when its roof falied to sup-port the brain tissues above. The ethmoid removal was a part of pansinusectomy per-formed on the patient for treatment of meni-ngitis on two occasions following a combined bilateral maxillary sinus and ethmoid opera-tions performed 13 months previously.

The patient was finally and successfully cured by fascia transplantation in the area of bony dehiscence and the use of antibiotic agents; the herniated brain tissues being fo-und partly atrophic and degenerated were sur-gically removed prior to the plastic cerebral procedure.
