経口蓋的萎縮性鼻炎手術

PALATINAL APPROACH IN OPERATIVE TREATMENT OF ATROPHIC RHINITIS

　Ⅰ．はじめに

　萎縮性鼻炎の治療には，多数の方法が挙げられているが，根原的な治療法は不明なためにもつぱら鼻腔の局所条件を改変することによつて病症の好転を図る鼻内成形手術がその主幹的な治療法となつている。手術の原則的方針は，固有鼻腔を狭くすることによつて鼻内気流に抵抗を与えて粘膜の乾燥を防ぐこと，あるいは萎縮した鼻粘膜を他の粘膜で置換することにおかれている。この方針にしたがつていろいろの手術方法が試みられているが，つぎのように大別することができる。

　（1）Lautenschläger氏法とその変法（固有鼻腔の側壁内転）

　In 4 cases of severe atrophic rhinitis the treatment was effected by insertion of siliconrubber into the submucous space of the nasal floor and septum by way of the palate, a method which is advocated by Professor Goto. Good results were obtained. Pictures of the nasal condition of the patient operated 1 year previously are presented.

　The author believes that this type of operation is especially adaptable when strong atrophic rhinitis is found and could be included concommittantly with radical sinus operations.
