舌根甲状腺腫を思わせたAdenoid Cystic Carcinomaの1例

ADENOID CYSTIC CARCINOMA OF LINGUAL RADIX

　Ⅰ．緒言

　舌根部に発生する腫瘍は全く多種多様で臨床的に決して稀なものではない。我々は一見恰も舌根甲状腺腫を思わせた腫瘍に対し組織学的検索を試みた結果，古来一名Cylindromaと呼ばれている"Adenoid Cystic Carcinoma"と判明した1例を経験したのでここに報告する。従来本疾患の治療に関しては手術的療法では腫瘍剔出に伴う手術侵襲の範囲について，放射線療法ではその是非について，しばしば論議の的となり種々問題点も多いのであるが，我々は本症例に対し腫瘍内に数回Radon Seedの刺入を行ない著明な効果を得て縮小せる残存腫瘍を手術的に容易に別出して全治せしめ得た。本疾患は組織学的には癌腫の範疇に入るものであるが，臨床症状は"良性混合腫瘍"に類似し極めて緩慢な経過をとるため，その治療法等に関しても今後の検討にまたねばならぬ点も多く，ここに報告する我々の症例も興味ある多くの示唆を与え得るものと考え諸賢の参考に供する次第である。

An adenoid cystic carcinoma the size of hen's egg invading the root of the tongue highlysuggested a thyroid tumor of that region. The diagnosis was established microscopically after surgical removal of the growth. Radon seed implantation was repeated 4 times with 10 seeds at a time. It has been previously reported that irradiation treatment in a simi-lar case was ineffective; yet, with present case this treatment proved to be highly effec-tive. The wound of the operation made a favorable progress in healing; a small tumor the size of the tip of the little finger which remained was snared off with a complete recovery of the patient.
