Wegener氏肉芽腫症—症例と文献的考察

WEGENER'S GRANULOMA AND REVIEW OF LITERATURE

　Ⅰ．緒言

　Wegener's Granulomatosisは臨床病理学的症候群で，その特色は気道（特に上気道）の進行性壊死性過程が先行するもので，之は組織学的に壊死性肉芽の像を示す。次いで全身撒布性の病巣（血管炎と巨細胞性肉芽巣）を生じ，その結果各種臓器に種々不定の症状を呈する。特に腎障害は殆んど全例にみられ死の帰転を辿る。現在の所未だ明らかな病原体或は発症機序は証明されておらず，恐らく過敏症過程が発病に関与する所大と考えられている。

　最近では本症の病像が明らかになるにつれて臨床的にも診断がつく例が少なくない。ここに最近我々は臨床症状及びその過程よりWegener's Granulomatosisと推定されるに到つた症例を経験したので報告し，併せて若干の考察を加え検討を試みたので諸賢の御参考に供したい。

　A case of Wegener's granuloma is reported. The patient was a woman, aged 29, who showed signs of necrotic scabbing of the nasal mucous membrane. She was placed under treatment for progressive necrotic rhinitis without response;the left eye became protruded, the joints were affected with rheumatic symptoms and signs of nephritis became apparent. A slight response was seen when Maruyama's vaccine and cortisone were used but seemed to be resistant to all other agents.
