鼻性化膿性髄膜炎を執拗に反覆した術後性鼻内脳ヘルニアにおける開頭手術例について

　Ⅰ．緒言

　近年化学療法の著しい発展から従来極めて難治と考えられていた鼻性髄膜炎症例にも多くの治験例が報告されて来た。その発生機序に関しては種種の原病巣ならびにそれに伴う感染経路が考えられ，鼻性髄膜炎が耳性のそれに比し予後不良の因もこの点にあるが，中でも特に脳ヘルニア合併症は臨床上重篤な経過をとり，しかもその根治療法としての手術々式の優劣は目下問題のある所で鼻内法，鼻外法を問わず今後の検討に俟たねばならない点が多い。我々は術後性と推定される右側篩骨洞天蓋より発する鼻腔内脳ヘルニアを伴い，1年3カ月の間に6回反覆して執拗な鼻性化膿性髄膜炎を併発し，この間その原病巣であるヘルニア門閉鎖の目的で経鼻的に脳硬膜の形成術を受けたが全て不成功に終つた難治の1症例に対し，右前頭部のUnilateral Craniotomyを施行しヘルニア門の骨穿孔部を頭蓋内より確認し，これを完全に閉鎖し手術的に根治せしめ得たのでここに報告し諸氏の参考に供する次第である。

A case of intranasal cerebral hernia is reported. The patient apparently had a radical sinus operation elsewhere consisting of maxi-llary and ethmoid sinuses. He was affected with, during the period of 15 months following the operation, persistent meningitis of nasel origin on 6 consecutive occasions. For this reason a unilateral craniotomy was performed, the artificial cranial opening was located; and, this opening was closed by means of spongel. The patient recovered without any further complication. The authors give warn-ings against any blind and violent procedures in nasal surgeries to save the patients from accidents of this nature.
