経上顎洞篩骨洞開放術創に対する病的上顎洞粘膜有莖移植法（赤池法）の臨床成績について

　Ⅰ．緒言

　慢性副鼻腔炎の手術療法はDesault, Küstnerらにより創案され，次いでCaldwellとLucによりその根治手術の基礎が確立された。以後我国では西端教授^1）の唱えるPansinektomieとなり現在に至つている。慢性複合性副鼻腔炎の中で最も多く発見されるものは上顎洞炎に篩骨洞炎の加わつたものであり，これらに対する手術法としては種々考えられているが，日常最も多く行われているものは西端教授のいわゆるKonservative Pansinektomieであろう。この手術を施行して術後経過不良の場合は篩骨術創治癒経過不良及びこれに起因すると思われる隣接部位罹患が最も多い。現在までの主流をなしている副鼻腔根治手術の主眼とする所の一つは病的粘膜の徹底的除去であろうが，これは細心の注意とかつ熟練により充分その目的を達し得ると思われる。しかし如何に手術を完全に行い得たとしても，術創を如何に順調に治癒せしめ，永久に排泄障害を生じぬようにするかということが，次に起る重大問題であろう。そのためにこれまで種々の後療法，薬剤などが考えられ実施されてきたが，その総てに充分満足し得る結果を得られてはいない。赤池は数年前よりこの点につき大なる関心を持ち続けてきたがある実例による着想から篩骨洞術創に上顎洞粘膜を移植することを考えつくにいたつた。これを実際の症例に施行し「経上顎洞篩骨洞開放術創に対する病的上顎洞粘膜有茎移植法」として第61回^2），第62回日本耳鼻咽喉科学会総会^3），及び耳鼻咽喉科^4）にその手術方法と簡単な成績を発表したが，私は今回，本方法を施行した50症例につきその臨床成績，特に手術時所見と治癒率の関係につき観察検討を加え，さらに片側移植例の両側治癒経過の比較，術後軽快に留まつた症例の検討及び長期観察症例の粘膜の病理組織学的所見などにつき検討し，いくつかの興味ある知見を得たのでここに報告する。

In 50 cases of chronic sinusitis consisting of operative procedures ethmoids were opened by maxillary sinus approach; in each operation the ethmoid bed was covei ed by a peduncula-ted flap of diseased mucous membrane turned upfrom the maxillary sinus. Following results were obtained: 1. The average healing rate was 81.8%; there was no difference in the rate of healing among cases where mucous membrane transplant was effected. 2. Wound healing was facilitated in cases transplant was effected as compared to those who were not. 3. Further studies were made among cases that failed to cure. 4. By observation of the transplanted mucous membrane for the period of 300 days postoperatively it was found that the results in different method of transplantation varied very little; the epithe-lium gradually changed to that of a celiated type but the gladular cells gradually disappe-ared as the interstitial inflammatory changes subsided.
