右側頸部における乳糜管の存在とその意義—頸部廓清術の経験より

　I．緒言

　頸部廓清術に伴う合併症の一つとして胸管損傷があり，その防止策として術前に脂肪食を与え，胸管を充満させ，明視確認しながら術を進めるのがよく，かつかくすればもし損傷しても乳糜の漏出により，その損傷個所を容易に結紮し得る事は我々が今迄主張してきたところである。

　一方左側頸部廓清術の際に胸管を損傷したとき若し胸管を完全に結紮した場合にはその副行路が当然問題となつてくるわけである。我々は胸管結紮後の副行路の問題を追求しているうちに，意外にも右側頸部静脈角附近にもかなり高い頻度で乳糜管が存在する事を発見したので，その所見を述べると共に，その意義について一応の結論を得たと思うのでここに発表し御批判を仰ぎたい。

In performing a right-sided cervical resect-ion following administration of preoperative fatty diet lymphatic duct was found in four cases out of the eight in the region of the ju-gular vein angle. The finding of such a duct in the right cervical region may be relegated to an abnormal thoracic duct or an establish-ment of collateral circulation following liga-tion of the duct in the left cervical region (which was established by experiments in dogs).
