頸部廓清術における胸管損傷とその予防策

　I．緒言

　頸部手術の際の胸管損傷は，従来よりリンパ節結核，悪性腫瘍のリンパ節転移の摘出，更に星状神経節切除等の際に時にみられていたのであるが，最近上気道及び食道入口部悪性腫瘍のリンパ節転移に対する頸部廓清術の普及に伴い，その際に生ずる胸管損傷が，その処置の煩しく困難であるのみならず時に不良の転帰をとる事もある事より，頸部外科に於て最近脚光を浴びて来た観がある。我々は先に第12回日本気管食道科学会総会及び「耳鼻咽喉科」第32巻12号に於て，胸管損傷を起した場合の処置について論じたが，その後の経験より左側頸部に於て胸管の確認は常に可能であることを知つたので今回は更にその損傷予防策について論じてみたい。

Kubo and Shirabe advocate the use of diet rich in fat preceeding cervical resection of the left side. By such a diet the authors ma-intain the thoracic duct will be made easier to identify and thus avoid its injury during the radical cervical resection.

The method of finding and recognizing the thoracic duct in relation to other anatomical strucures is dicussed.
