防音設備のない検査室に於ける聴力検査成績と複合ストレプトマイシン週2回筋注法による聴力像の影響に就て

　1．緒言

　ストレプトマイシン（以下SM）系薬剤による化学療法が結核治療の主流を占めている事は今もなお周知の所であるが本剤投与中に起る副作用としての耳鳴，めまい，顔面硬化感等が，稀に遭遇する恢復不能な聴力損失に対して患者に若干の不安を与えているようである。

　いわゆるSMによる聴力損失（SM難聴）は果してどの位起るものなのか。又高度の難聴にならないで軽度或いは中等度の難聴位で停止するものがあるものなのか^10）。又或る学者はSM難聴は極めて少数の例外的な副作用であるから非難するに当らぬと述べている^7）9）一方，他の学者はSM難聴には潜在性のものもあり，割合多いものであるから定期的聴力検査が必要であると結論している。実際文献を一読するにSM難聴を多いものとする報告の方が多い^{1）3）4）11）12）13）14）15）16）17）}。又常に安静を守つている療養所の方が安静を守れない外来患者より副作用が少ないとする学者もある。我々は結核予防会第一健康相談所に於いて外来化学療法を施行しているが，之等の問題につき検討を加え，若干の知見を得たのでこゝに報告する。

The results of audiometry, using JIS stan-dardized instrument, conducted in an ordinary room of 50 to 60 phon noise on one hand, and in a noise-proof room of 20 to 30 phon noise on the other, are compared. The material of the study consisted of patients who were un-der treptomycin therapy of bi-weekly admi-nistration. It is revealed that if the audio-metric tests, even though they are conducted in an ordinary room of 50 to 60 phon noise, were to be continued for the period of 3 to 5 months in duration the capability in detection of streptomycin-deafness will not be decrea-sed. Among about 200 cases observed the inci-dence of streptomycin-deafness appeared to be 1% involvement. There seemed to be two ty-pes of involvement in streptomycin-deafness, one in which the patient Will be affected with rapid onset of deafness that may even include the low-tone area, when the total amount of streptomycin administered might be 10 grams or less, and, the other the deaf-ness may be limited to high tone areas alo-ne, when the administration reaches 50 g or over.
