はしがき
本症例は当教室の経験例でなく,当教室関係者で開業している共同発表者橘田氏の経験例である。大病院におけると異なり,いろいろ検査事項に不十分な点があると思うが,こういう予期せざる事態に対して,開業せるものとしては,相当検査が行届いている事は次の報告にある通りである。
私共が此の症例を取り上げたのは,術中の不慮の急変に対処し,どの程度まで生命を救い得るかという私共の研究の資料になるものではないかと考えているからである。
A case with an onset of an acute pulmonary edema during the course of establishment of conduction anesthesia on the facial branch of the trigeminal nerve is reported. The patient, a man aged 18, was prepared for sinus operation and when novocain anesthesia of the infraorbital nerve was begun his face turned to be extremely pale and cyanotic; the breathing became labored which finally changed into coughing producing frothy sputum mixed with bright red blood. By way of treatment oxygen inhalation was immediately established; wintermin, gronsan, dextrose solution and noradrenalin were given by means of subcutaneous and intramuscular injections and also by intravenous drips. The patient recovered.
Further, in experimenting with guinea pigs the authors succeeded in producing congesiion and hemorrhages in the internal organs by application of hyperstimulation to the infraorbital nerve. They believe that this fact may have some bearing in connection to the symptoms that were met with in the patient reported above.
