篩骨洞への開放による涙嚢炎手術例

　緒言

　慢性涙嚢炎は鼻涙管狭窄に二次的に化膿菌の感染によつておこるとせられているが，又涙嚢の感染のために鼻涙管の狭窄を来すもの，又涙嚢の拡張を来して涙液の排泄障碍を来し更に病症の悪化を招くもの等がある。その治療としての根本方針は涙嚢の除去か，鼻涙管の流通障碍の矯正にあると見ることが出来る。

　涙液の下鼻道への生理的な排泄が障碍された場合に鼻腔の前側壁に涙嚢から直接に新な交通路を設けようとするのがTotiの手術である。然しTotiの手術には又欠点を有しているために広い普及を見ずにいる。今日行われている方法は概ねDupuy-Dutemp法及びその変法であつて好結果の得られるものも少くないが，尚難点を蔵して満足な手術とは見做されていない。そのために最近はResintubeの挿入等の方法が案外普及したような結果になつた。この方法も結局挿入管口の肉芽による閉塞のために反省期に入つて新な方法の出現が望まれている。この方面の手術として三井，生井，大橋，上岡等の諸氏の報告がある。最近耳鼻科の雑誌にもGood yearのDacryocystorhinostomyに就ての小論文ありてこの種の手術は鼻科学的な知識の導入が必要に思われる。著者等は長大耳鼻科後藤敏郞教授の行つている方法に従つて涙嚢の開口を篩骨洞に設けることを主眼にした手術を行つて好結果を得たのでわれわれの小経験を報告する。

　Kamemoto states that, for treatment of chronic dacrocystitis, operative method of Toti or its modifications is generally the accepted means. The underlying principle of this method is based on creating an open passage between the sac and the nasal cavity that will stay patent. However, the method is not altogether devoid of faults nor are its modifications which prevented their becoming popuat any time.

　In recent years Goto of University of Nagasaki Medical School devised a method by which the drainage channel of the sac to be diverted with its opening into the ethmoid sinus. The treatment of chronic dacrocystitis by this method in the author's has been highly satisfactory.
