上顎洞腺腫の1例

　緒論

　腺腫に就いては，1845年Lebertの報告を以つて嚆矢とし，爾来比較的多数の報告があるが，その発生部位としては，胃，腸，子宮に最も多く，その他，乳腺，卵集，甲状腺，肝，腎，粘液腺，唾液腺，涙腺，睾丸，前立腺，膵，肺及び皮膚腺等にも見られるという^22）。我が耳咽喉科積域に於いては，鼻腔及び副鼻腔の真性腺腫は稀であるとされている^27）。文献を徴するに，外国に於いては26例，本邦にては8例の報告例があるのみである。而してその発生部位としては，篩骨洞及び鼻中隔粘膜が最も多く，その成因としては不明であるが，一般には先天的のものとされ，成長は可なり緩徐で，仮令不完全な手術的操作を加えても，これによつて特に増殖することはなく，其の臨牀症状としては，漸次に増強する鼻呼吸障碍，時に鼻出血が挙げられている^6）27）。

　最近，私達は上顎洞に発生した該腺腫の一例を経験し手術を施行し得たので，其の発生部位，臨牀症状等を報告し，文献症例として追加致したいと思う。

Yamada and associates report a case cf maxillary sinus adenoma that produced symp-toms indistinguishable from that cf acute sinusititis with impaired nasal breathing and swelling of the face on the side of involve-ment.The diagnosis of the case as being an adenoma is established by microscopic exami-nation which proved the growth to be that of bemgn character. The X-ray examination, however,showed a considerable bony destruction of the sinus wall from the surface of which the growth evidently made its start and, from clinical point of it must be considered as having the earmarks of malignancy.
