はじめに

　verrucous carcinoma（VC）は1948年にAckerman^1）によって確立された扁平上皮癌（squamous cell carcinoma：SCC）の亜型である。通常のSCCとは異なり局所での緩徐な発育や弱い浸潤性，低い転移率，外科的切除により予後良好などと特徴づけられる低悪性度の癌であり，その頻度はSCCの2.2％といわれている^2）。頭頸部領域では口腔や喉頭に好発するが鼻副鼻腔の発生例は稀で，すべての鼻副鼻腔腫瘍の1％程度との報告がある^3）。今回われわれは，再発を繰り返し診断に難渋した上顎洞VCの1例を経験した。本症例についてその臨床像を提示し，文献的考察を加えて報告する。

　Verrucous carcinoma（VC）is a rare tumor in the sinonasal tract. We reported a case of VC of the maxillary sinus.

　A 28-year-old Japanese male presented with a one-year history of left-sided nasal obstruction. The wall of trouble side was swelling in the inferior nasal meatus. CT demonstrated an expansive soft-tissue lesion in the left maxillary sinus. The structure of this tumor seemed like a boiled egg. We performed operation three times because of recurrence. After the last surgery definite diagnosis was pathologically confirmed as VC finally.
