上咽頭惡性腫瘍の2型—附 本邦文獻に現われたる上咽頭惡性腫瘍の臨床的統計的観察

　緒言

　上咽頭惡性腫瘍とは元來上咽頭腔に原発する惡性腫瘍を意味するものであるが，一般には頭蓋基底部に生じ上咽頭腔に現われたる惡性腫瘍も之に含めて老えられている．即ち上咽頭に腫瘍形成のあるという点で両者は一括して所謂上咽頭惡性腫瘍と呼ばれているのであるが，前者にては上咽頭腔即ち原発地であつて上咽頭病変はそのままその初期病像を形作るのであるが，後者にては上咽頭病変は單にその経過中の1病像に過ぎないのであつて，兩者は嚴密には区利さるべきである．

　然しながら斯る患者が医を訪れる場合，この2つのものの間に余り差はないのであつて．又臨床上明確に之を区別することの困難なことも少くないので，所謂上咽頭惡性腫瘍なるカテゴリーに於て之を述べることは妥当と考えられる．

　Sakamoto notes observations on two cases of neoplastic growths that occurred in the upper part of the larynx in accordance to their two distinct forms of progress. One of these an endothelioma is found in a woman 47 years of age and, the other, an epithelioma in a man 49 years old. In the first case the spread of the growth had taken a general direction towards the skull base and postnasal cavity to manifest symptoms in types of Pseudo-Gradenidinigo syndromes which later included paralysis of the optic nerve. In the second case the spread was directed more to the anterior portion of the nasal cavity. The growth which started to spread from the right side of the upper part of the larynx manifested itself in the right paranasal sinuses resembling a carcinoma of this area.

　The author cellects from the literature of this country sixty eight other cases of neoplastic growths that occurred in the upper part of the larynx and attempts to review them from clinical, statistical standpoint.
