Grunert氏手術法の簡便化に就いて

　序言

　Grunert氏手術法は周知の如く脳靜脈洞血栓がS状洞から頸靜脈球，頸靜脈に及んだ場合に行ふもので，非常に面倒で時間のかゝる手術である．その主眼はS状洞から頸靜脈球，頸靜脈を一連の帶として露出して之を切開して血栓を剔出する所にある．

　それ故血栓除去の點では最も徹底しているが手術が面倒で時間をとるから乳樣蜂巣鑿開手術や中耳根治手術を行つてから引き續きこの手術を行ふ事は實に大きい勞力を要する．それ故この點を幾分でも簡略にしたいと云ふ事が第1．

In an emergency operation, on a patient, a woman with a weak heart and at the time preg-nant, for an extensive cranial sinns thrombosis by Grunert's method, the author caused an acci-dental section of facial nerve. In another case, a man 50 yrs of age was also operation by same method. But soon afher the operation this patient developed hemorrhage from the te-mporal region which was continuous and unde-rneinable in origin, and the care of which req-uired 6 months of continnual attention. In view of the difficulties encountered in thesecases, when Grunert's method of operation was employed, the author devised a procedure which simplified the original method. Removal of the thrombus is made without exposing the superior jugular bulb, but from two approaches, namely, sgnmid siuns and internal jngular vein, with the use of author's specially constructed forcpes for the pur-pose, three cases brought to cure by this method are presented with full discussion as to the simp-lification of the method. Procedural difficulties met in Grunert's method are fully analysed.
