喘息發作樣呼吸困難を呈した喉頭ヂフテリアの1例

　緒言

　由來下氣道ヂフテリアは診斷困難なものゝ一つに數へられて居るのであつて，而も緊急を要し，且つ其の豫後は上氣道ヂフテリアに比し甚しく不良である點を注意せられて居るものである．

　其の鑑別診斷として重要なものは假性クループ，下記道異物，肺炎等が擧げられてゐるのであるが，病歴の詳細な調査，細菌檢査，X線檢査等により多くは判斷し得るのであるが，其の決定的診斷を與へるものは直達鏡檢査法である．

　余等はさきに山形縣鶴岡市庄内病院に勤務中，一見喘息發作樣の呼吸困難を呈し，直達檢査を行ふには餘りにも全身状態が衰弱して居り，一時診斷に迷つた症例に遭遇したのでこゝに其の概要を報告する．

A boy of 6 years and 9 months old developed expiratory dyspnea with signs of pulmonary em-phsema 10 days following antidyphtheritic inno-culation. Adrenalin and Ephedrin administrations were of no avail. Pseudomembrane was found in the subglottic area and B. Dyphtheria was positive on the laryngoscopically removed specimen. P atient developed pneumonia and died.
