惡性ヂフテリアに就いて—ヂフテリア症1092例の治療と反省

　福島女子醫專耳鼻科教室で昭和8〜23年の16ヶ年間に取扱つたヂフテリア症1092例の總括であるその間の新來患者實數81413名であり，ヂフテリア症は1.2％に當る．死亡は62名，5.6％となる．

　昭和8〜13年の間は各年度親患數も餘り増減なく，ヂフテリアそのものも，新患數の増減に比例しての數の變化を見て，新患に對するヂフテリア數は0.3〜0.6％に止まる．ただ13年頃より破調の萌しがあつた．

Iinuma found 62 deaths or 5.6 % among the series of 1092 cases of diphtheria that had been diagnosed and treated, since 1933 to 1948 inclusive, at the Otolaryngological Department of the Fuku-shima Women's Medical College. The route of in-vasion in these cases are as follows: Eyes, 2; ears 4; bronchi, 120; nose, 153; larynx, 249; and, pha-rynx, 862 cases. Thirty six cases are found of malignant diphtheria, the 19 of which are met with death.

Causatige agents for the malignant type may not be, blamed to the presence of corynebacterium diphtheriae alone, but other bacilli such as stre-ptococcus homolyticus should be regarded with much weight. The use of diphthria antitoxin is without question the prime importance, but the therapy directed against the malignent type of diphtheria calls forth supplementary use of other agents such as, penicillin, sulfa-derivitives, cardiac tonics, vitamin B conpounds, and, antistrepto-coccic serums with equal.
