Dimethylsulfoxide（DMSO）を用いた冷凍保存自家骨による頭蓋形成術の検討

Ⅰ．はじめに

　外減圧術後の頭蓋形成術の際に使用される骨欠損部の補填物質としては，methyl methacrylateやセラミックなどの非生体材料か，または生体材料（自家骨）を何らかの方法で保存し使用するかに分けられる．自家骨が使用可能であれば，これに勝る理想的な材質はないという考えは諸家の認めるところであり，自家骨を保存する方法としては自己の腹壁や大腿部の皮下に埋没，保存する方法^17），冷凍保存する方法などが報告さている．特に冷凍保存に関しては，古くは1951年Elliott ＆ Scottt^2），1952年にOdom^18）により，本邦では1967年宮崎^15）により報告され，以降も報告例が散見されるが，術後感染や骨融解，骨吸収の問題が指摘されている．

　われわれは，冷凍保存のみで強い骨融解で骨弁が完全に吸収され人工骨による頭蓋形成を余儀なくされた2症例の経験から，骨融解，骨吸収を予防する目的で，凍結保護剤であるDimethylsulfox-ide（以下DMSO）を用いた冷凍自家骨による頭蓋形成術を実施してきたので報告する．

It is generally agreed that the autogenous bone flap which has been removed at the time of external decompre-ssion would be superior to any artificial material if it can be used in cranioplasty. Cranioplasty using autogenous fro-zen bone graft has been reported and showed good results except for infection and severe bone absorption. We conducted 39 cases of cranioplasty with cryopreserved autogenous bone in the presence of 10% of Dimethyl-sulfoxide (DMSO), which has been reported as a cryoprotective agent.
