出血で発症した舌咽神経鞘腫の1例

Glossopharyngeal Neurinoma Causing Symptomatic Hemorrhage: Case report

I．はじめに

　腫瘍内出血をきたす頭蓋内腫瘍としては一般に神経膠腫や下垂体腺腫などがよく知られているが，symptoma—ticな出血をきたす神経鞘腫はまれと考えられ，文献的にも15例の報告を見るのみである^{1-3,5,6,9,10,12,14-18,20）}．われわれは大量の出血を契機に診断された舌咽神経鞘腫の1例を経験したので，その出血機序などを中心に考察を加え報告する．

A 86-year-old man was admitted to our hospital be-cause of swallowing disturbance and deterioration of consciousness. He had been aware of hearing distur-bance on the right side for twelve months. Computed tomography (CT) scan demonstrated an inhomo. geneous hyperdense mass lesion, 3.2×2.3cm in size, at the right cerebello-pontine angle, and ventricular en-largement with intraventricular hemorrhage. Skull tomogram revealed destructive enlargement of the right jugular foramen.
