I．はじめに

　頭部外傷により，小脳症候を呈することは比較的稀である．その頻度は，0.4-0.7％と言われている^7,12）．もし，この様な患者を診た場合には，後頭蓋窩血腫をも疑う必要があると言われている．頭部CTの普及した今日では，後頭蓋窩血腫の診断は，決して困難ではない．しかしながら，小脳症候の検査が可能な程度の，意識障害の比較的軽い頭部外傷患者の場合には，その患者が小脳失調を呈して，小脳の挫創が疑われても，実際にCTで小脳の異常が証明されることが少ない，ということもまた事実であるように思われる^8）．

　MRIは，骨のアーチファクトによる影響をうけないため，特に後頭蓋窩の観察に，その威力を発揮する．また，CTでは検出されない様な，小さな脳挫傷や脳浮腫を，MRIは描出することができるということも知られている^2,3,5,10）．

Abstract

Four cases of cerebellar dysfunction following head trauma are presented. Cerebellar signs revealed were those such as dysmetria, dysdiadochokinesis, horizontal nystagmus and ataxia. T2-weighted magnetic resonance (MR) imaging (0.15tesla, spin-echo method; TR 2000msec. and TE 100msec.) revealed focal lesions in these patients, although CT scan failed to demonstrate any changes in the cerebellum.
