急性硬膜外血腫の頭蓋内合併外傷についての検討—MRIによる評価

MR Imaging of Associated Brain Injuries in Cases with Acute Extradural Hematoma

I．はじめに

　急性硬膜外血腫（EDH）は，よく経験される外傷性頭蓋内血腫のひとつである．本症はCTにより迅速かつ正確に診断され，適切な治療により，その救命率および神経的機能予後は向上している．しかし，頭蓋内に合併外傷が存在する場合には，それが手術後脳腫脹や外傷後けいれんの原因となることもあり，EDHの治療を行ううえで合併外傷の評価は重要である^{4,9,11,16,17,19,21）}．EDHの合併外傷についての従来の検討は，おもにCTを用いてなされたものであるが^3-5,17），非出血性病巣の場合にはCTでは描出されないこともあり，合併外傷についての適切な評価のうえで問題も指摘されている^{5,7,8,10,14,20，22-24）}．

　われわれは，EDH症例の頭蓋内合併外傷の検索におけるMRIの有用性および問題点を評価することを目的に，臨床経過，CT所見，脳波所見とMRI所見との比較検討を行った．また，急性期症例に適した撮像パラメータ，撮像中のモニタリングおよび呼吸循環管理についての考察も行ったので報告する．

To assess the efficacy of magnetic resonance (MR) im-aging for detection of associated brain injuries in cases with extradural hematoma (EDH), 32 patients of EDH were examined by MR. CT detected associated lesion in eleven patients (34%), while MR detected them in 24 patients (75%). MR is more sensitive than CT in detect-ing associated lesions, especially when T2 weighted im-aging is used. Non-hemorrhagic contusions adjacent to EDH, and near the cranial base were well shown by MR, however they tended to be missed by CT. EEG findings were dearly related to abnormalities detected by MR. Coupling between functional change and organic change was confirmed.

The improved detection and anatomic localization of associated brain injuries by MR should allow more accu-rate assessment of brain injuries, and sophisticated man-agement of EDH patient.

The authors also discussed on the cardiorespiratory monitoring and support during MRI examination in criti-cally ill patients.
