内頸動脈硬化性変化に伴う視神経Vascular compression neuropathy

Vascular Compression Neuropathy of the Optic Nerve by Sclerotic Internal Carotid Artery

I．はじめに

　脳腫瘍，水頭症，動脈瘤など視交叉部病変が視力視野障害を呈することはよく知られているが，稀に硬化性変化により蛇行，拡張した内頸動脈により視神経が直接圧迫され同様の症候をひき起こすことが報告されている．こうした視神経のvascurar compression neuropathyは，診断技術の進歩した今日でも術前の確定診断は必ずしも容易ではない．したがって，手術時にはすでに視神経萎縮を呈しているものが多く，手術成績も良好とはいえなかった．われわれは，本症の神経放射線学的診断法を工夫し，視束管開放術とともに，内頸動脈転移などにより視神経に対し直接的減圧を試みた3症例を含む4症例につき，過去の文献例を加え報告する．

Although mechanical compression of the optic nerve by a dilated and/or tortuous sclerotic intracranial inter-nal carotid artery is occasionally reported as an etiolo-gy for impairment of vision and visual field, early di-agnosis of such condition has been hampered by the lack of characterstic neuro-ophthalmological and neuro-radiological findings.

The review of 35 previously reported cases (60 eyes)including ours showed that 41% out of the 34 operated eyes were improved neurologically by the decompres-sive procedure, while it was in 27% out of the 26 un-decompressed or non-oprerated eyes.
