頭蓋内主幹動脈閉塞急性期症例に対するウロキナーゼ動注療法の試み

Intraarterial Urokinase Infusion Therapy for the Acute Intracranial Major Artery Occlusion

　I．はじめに

　閉塞性脳血管障害急性期症例に対して行われているUrokinaseを用いた線溶療法は，その効果について明確な評価を得ていない^2-4,7）．一方，心筋梗塞急性期症例に対する冠動脈内Urokinase注入療法（percutaneous trans—luminal coronary recanalization：以下PTCR）は閉塞血管の55-88％を再開通させ，着実に効果を上げつつある^6）．われわれは最近，頭蓋内主幹動脈閉塞急性期3症例に対し，Urokinase動脈内注入療法を試み，2例に著効を得たので，若干の文献的考察を加えて報告する．

Recently percutaneous transluminal coronary recana-lization therapy (PTCR) with urokinase infusion has became one of popular technique for coronary arterial occlusion. This paper reported clinical experience of in-traarterial urokinase infusion therapy for acute or su-peracute stroke patients. The procedure was followed by angiographical study which revealed the major intracerebral arterial occlusion in three cases.

Case 1 : A 74-year-old female had sudden onset of clouding of consciousness with complete left hemiple-gia.
