頭蓋顔面骨折の治療

Treatment of Craniofacial Fractures

I．はじめに

　脳神経外科に搬入される患者の中には受傷機転から顔面外傷，顔面骨折を合併していることも稀ならず経験する．受傷時顔面の腫脹やその他の症状に隠蔽され，的確な骨折の診断が下されなかったり，また頭蓋内損傷合併例では脳外科的処置が急務であるため顔面骨折の適切な治療時期を失することも稀ではない。著者らはこのような点を鑑みて，頭蓋顔面合併損傷例に対し，脳外科的治療と並行して早期に顔面骨折の治療を行い良好な結果を得たので，その診断，病態ならびに手術方法について報告する．

In the facial fractures, a large number of these pa-tients also sustain a head injury. Neurosurgical care should be the first choice when an intracranial injury is associated. In such situation, suitable timing of the re-duction of the facial fractures would be often missed and this would result in functional disturbance and de-formities of the face. The present report concerns the early definitive treatment of the extensive craniofacial fractures. Operative procedure and its result are pre-sented.

We have surgically treated 100 cases with facial frac-tures during the past 7 years.
