CT上脳室内にFree fatを認めた松果体奇形腫の1症例

A Case of Pineal Teratoma with Intraventricular Free Fat on CT Scan

I．はじめに

　頭蓋内奇形性腫瘍，特に類上皮腫では，腫瘍被膜の自然破裂，あるいは摘出手術に際して，刺激性の強い脂質成分が脳室内あるいはくも膜下腔に拡散し，組織反応性の著しい肉芽腫性髄膜炎を呈することはよく知られており，頭蓋単純撮影にて腫瘍内あるいは脳室内にfat-fluidlevelを形成することがある^2,9）．しかし，皮様嚢腫，奇形腫を含めCT上free fatとして描出されることは，極めて稀なものである．われわれは，経過中CT上，側脳室前角内にfree fatを認めた松果体奇形腫を経験したので，神経放射線学的所見とともに症候学的特徴について，文献的考察を加え報告する．

Detection of an intraventricular or intratumoral fat-fluid level on the plain craniograms has been known as a characteristic sign indicating the presence of intracranial teratomatous tumors. On CT scans, however, only thirteen cases have been previously reported to be found an intraventricular and/or sub-arachnoid free fat associated with spontaneous rup-tures of these tumors. We reported a case of pineal teratoma with intraventricular free-fat seen on CT scans.

A nine-year-old male with precocious puberty was admitted to our hospital complaining a moderate nonpulsatile headache.
