高血圧性視床出血に対する定位的血腫溶解排除術

Stereotactic Evacuation of Hypertensive Thalamic Hematoma using Plasminogen Activator (Urokinase).

I．はじめに

　高血圧性脳出血の外科的治療は，従来から主として被殻出血に対して開頭術により脳内血腫除去術が行われてきた．

　視床出血や中脳出血に対しては手術効果に比して手術侵襲が大きく手術適応となり難かった．しかし，手術侵襲さえ極力小さくすることができれば血腫を除去することが望ましいことは論をまたない．われわれは1978年より手術侵襲を少なくするために定位脳手術法を用い，硬い凝血を吸引するためにplasminogen activatorであるUrokinaseを用いて血腫を溶解する方法を創案し，160例の高血圧性脳出血に優れた成績をあげてきた^{3,4，11,12）}．今回はこのうち視床出血44例の手術成績について報告する．

In the past five years, the authors have performed stereotactic evacuation of hypertensive thalamic hema-toma using Komai's CT-stereotactic apparatus in 44 cases. Liquefied hematoma was aspirated through a stereotactic cannula, and solid hematoma difficult toaspirate was dissolved by a plasminogen activator (Urokinase) and drained out through a silastic catheter.

The mean ratio of the total volume of evacuated hematoma to the estimated volume by CT image was 83.8%.

The recovery from motor paresis and consciousness disturbance was observed during the early postopera-tive days in most patients.
