I．はじめに

　聴性脳幹反応（auditory brainstem response，以下ABR）は，聴覚系および脳幹部の機能的変化を把える有用な検査法であるのみでなく，後頭蓋窩手術の術中モニターなど数多くの臨床応用に関する報告がある．われわれは，ABRを高血圧性橋出血における予後推定と，その手術適応の決定に利用しているが，時にはCT上の血腫部位と，ABRの所見に，障害側の一致をみないというdiscrepancyを経験する．この点を解決するとともに，ABR諸波の起源の異常と脳幹部血腫との関係を明らかにするために血腫と脳幹部聴覚伝導路を三次元的に立体構成した．それらの位置関係とABRの潜時及び波形変化を血腫除去術前後で比較検討したので報告する．

A three-dimensional reconstructed display using a microcomputer represents the spatial correlation be-tween the nuclei of auditory tract and pontine hemato-ma. Stereotaxic evacuation of hematoma was per-formed on seven cases of pontine hemorrhage. By mak-ing a comparison between preoperative and postopera-tive findings of ABR and this display, we examined the effect of the surgical intervention on ABR findings.

ABR was recorded according to the standard way (100 dBSL, 2000 click stimuli). Hematoma was drawn on the Atlas of the Human Braisnstem and CerebellarNuclei by Farhad Afshar.
