I.はじめに
転移性脳腫瘍の発生頻度はCTスキャンの普及および悪性腫瘍患者の生存期間の向上とともに増加する傾向にあるが,その治療に関しては依然として困難な問題が横たわっている.
私どもの教室で脳腫瘍に対する外科的治療を開始して以来,1982年12月までの過去15年間に扱った全脳腫瘍症例は850例で,うち転移性脳腫瘍は85例で,10%を占めている.今回,私どもはこの85例についてその治療成績を検討したので,その結果を述べるとともに本疾患に対する外科的治療の現況と将来の展望に関して若干の考察を加え報告する.
Results of surgical treatment in 85 cases with metastatic brain tumors are reviewed. The lung was the most frequent site of primary lesion and the following sites were GI tract and the breast. Adequate treatment consisted of total removal of tumor, irradia-tion and/or chemotherapy were carried out in 51 cases. The remaining 34 cases had an unsuccessful treatment because of their poor physical condition. Mean survival time after adequate treatment was 8.75 months in the former group and 3.06 months in the latter group. Of 51 patients (86.3%) in the former group, 44 showed improvement of the neurological signs after treatment.
