I．はじめに

　一般に大後頭孔"前半部"に位置する硬膜内腫瘍の摘出には多くの困難を伴う．microsurgery導入以後，後方到達法にて全摘出可能であった症例が稀に報告されているが^11,13,25），その危険性は高く，部分摘出にとどめざるを得ない場合が多い．

　今回われわれは，解剖学的に最も到達が容易である経日的前方到達法を用いて，大後頭孔前半部に位置する硬膜内腫瘍を全摘出し，良好な結果を得た症例を経験したので文献的考察を加え報告する．本症例は，渉猟し得た範囲では，腫瘍摘出後，硬膜形成ならびに前方骨固定を併せて施行し得た最初の報告例である．

A case of an anteriorly located meningioma in theregion of the foramen magnum, successfully treatedby a transoral approach, is reported. A 60-year-oldwoman was admitted to the author's department onJanuary 19th, 1983. Six years before admission, shebegan to experience the gradual cold dysesthesia inthe both palms, and then developed gait disturbance,clumsiness of the right hand and a stiffneck. Shewas found to have atrophied hands, mild spastic tetra-paresis, ataxia and hypalgesia under the bilateral C2spinal levels.
