大後頭孔腫瘍—診断と手術アプローチ

Foramen Magnum Tumor: The diagnosis and surgical approach

I．はじめに

　大後頭孔腫瘍はAbrahamsonとGrossman^2）が最初に報告して以来，現在まで多数の症例報告がなされているが，この部位の腫瘍は多彩な臨床症状を示し，いわゆる"foramen magnum syndrome"を呈することが知られている．この症状は一般に進行性であるが，稀ながら症状軽快が起こることがあり．神経症，変形性頸椎症，多発性硬化症などと誤診され，かなり病期が進行するまで適切な治療を受けない場合がある．Love^19）らによれば，大後頭孔腫瘍のうち約30％は良性，髄外腫瘍であり，全摘出が可能であり，早期診断，治療が予後の向上に重要であると考えられる．従来の同部腫瘍の文献的報告は髄外腫瘍に限定されたものが多く^{5,8,9,15,24,26,28）}，髄内腫瘍との対比で論じた報告は少ない^12）．

　今回，著者らが経験した大後頭孔腫瘍7例の臨床症状について検討し，診断学的考察を加え，さらに手術アプローチに関して若干の検討を加え報告する．

The seven cases of the foramen magnum tumors were presented with the clinical manifestations and surgical consideration. Early clinical symptoms of the cases with extramedullary lesions were subocci-pital neck pain followed by dysesthesia, clumsiness of hand and weakness. On the other hand, early symptoms of the cases with intramedullary lesions were dysesthesia. often followed by swallowing diffi-culty or hoarsness, which may have some difference from the clinical course of the extramedullary tumor cases.

CT scan was remarkably useful in the diagnosis of the foramen magnum tumor.
