 A 70-year-old woman was admitted to our hospitalwith attack of SAH. On admission, she was semicomatose with no other neurological deficit.

 Left carotid angiography revealed an aneurysm ofthe anterior communicating artery and a large abnormalvessel connecting the left external carotid artery andthe left vertebral artery. This large anastomoticvessel was thought to be primitive proatlantal intersegmental artery (proatlantal artery I). Right carotidangiography revealed a coiling of the right internalcarotid artery and hypoplasia of the right Ai portion.
