脳動脈瘤を伴ったPersistent primitive proatlantal intersegmental artery（Proatlantal artery I）の1例

A Case of Persistent Primitive Proatlantal Intersegmental Artery (Proatlantal Artery I) with An Aneurysm: A case report

I．はじめに

　脳動脈瘤，特に嚢状動脈瘤は一般に先天性のものであるといわれているが，必ずしも異論がないわけではなく，その発生原因は明確に把握されていない．今回われわれは胎生期の遺残血管であるpersistent primitiveproatlantal intersegmental arteryを伴った破裂脳動脈瘤の1症例を経験し，脳動脈瘤発生原因との関連を示唆すると思われたので，特に遺残血管と脳動脈瘤の関係を中心に文献的考察を加えて報告する．

　A 70-year-old woman was admitted to our hospitalwith attack of SAH. On admission, she was semicomatose with no other neurological deficit.

　Left carotid angiography revealed an aneurysm ofthe anterior communicating artery and a large abnormalvessel connecting the left external carotid artery andthe left vertebral artery. This large anastomoticvessel was thought to be primitive proatlantal intersegmental artery (proatlantal artery I). Right carotidangiography revealed a coiling of the right internalcarotid artery and hypoplasia of the right Ai portion.
