前交通動脈瘤に対する手術アプローチへの考察—112例の脳血管撮影，手術所見から

Some Considerations on Surgical Approaches to the Anterior Communicating Artery Aneurysms from the Radiological Study of 122 Cases

I．はじめに

　前交通動脈瘤の手術アプローチとしては，現在pterional approach^12,13）をはじめ，interhemispheric ap-proach^4,7），bilateral frontal approach^6,10），subfrontal ap-proach^5,9）など種々のアプローチがとられている．当施設においては，主としてpterional approachを採用しているが，その際にも左右どちらから進入するかは，術前に決定しなければならない大きな問題である．今回我我は過去5年間に当施設において手術を行なった前交通動脈瘤について，術前の脳血管撮影および手術記録による考察を行なったので，当施設における前交通動脈瘤に対する手術アプローチ決定の考え方について報告する．

We studied the operative approaches to anteriorcommunicating artery aneurysms by means of review-ing preoperative angiogram and operation records.During the past 5 years, 117 patients with anteriorcommunicating artery aneurysms were operated on inour hospital. Among these, we examined 112 casesof which preoperative angiogram and operation recordswere satisfactory for this study. We checked the nextpoints on the preoperative angiograms. 1. Neck ofaneurysm-planum sphenoidale distance. 2. Directionof the anterior communicating artery.
