I．はじめに

　近年，超音波診断機器の発達により，開頭術中の超音波診断が再び試みられるようになってきた^1,2,5-13），その応用は，現在までに脳腫瘍，脳内出血，水頭症などの手術について多く報告されているが，脳動脈瘤については，その部位や大きさの問題（すなわち，頭蓋底に近いため，直径10mm以下の小さなものが多い動脈瘤そのものの情報としては明白なものが得られにくい）もあるためか，付随した病変である脳内血腫や水頭症を伴っている場合を除いては，ほとんど応用されていないのが現状である．われわれは最近，超音波断層法による術中診断を32例施行したが，その結果，現在までに報告された有用性を十分確認することができた．さらに脳動脈瘤そのものについての情報も，large aneurysmの症例をはじめとして，有用な経験をいくつか得ることができたので，あわせて報告する．

The authors' experience with the application ofreal-time ultrasonography during 29 and after 3neurosurgical operations is reported. Intraoperativesonography was performed in 29 cases. The neuro-surgical operations include aneurysm surgery in 13cases, removal of intracerebral hematoma in 11 cases,neoplasm surgery in 2 cases, and so on. Moreover,sonographic examination through hone defect wasperformed in 3 cases after decompression craniectomy.
