サイラスティック代用硬膜直下にみられた術後血腫

Formation of a Hernatoma under Silastic Dural Substitute

I．はじめに

　代用硬膜は硬膜欠損部の補充，あるいは外減圧術の際に用いられることがあるが，これを用いたために生ずる合併症^2,3,9,11）も少なくない．

　現在用いられている代用硬膜の中でサイラスティッケ（Silastic sheetingシリコンゴムシート，Dow Corning社，U．S．A．）硬膜補填材は表面平滑で可撓性があり操作しやすく，水を通さず，さらに水密に生体硬膜との縫合が可能であり，また膜自体の変化や，周囲組織との癒着もない，などの利点があり，われわれもしばしば用いてきたが，手術の数日後にサイラスティッケ代用硬膜直下に，その形状に一致した限局性の凝血塊ないしは血腫の発生をみるといった合併症を数例ならず経験した．

Several cases of spontaneous hematoma formationoccurred under the Silastic dural substitute werereported with CT-scan and recraniotomy findings.

In overall analysis, an incidence of thespontaneoushematoma formation was 35.1% in cases of artificialdural grafting with Silastic sheeting and none withautogenous temporal fascia.

On CT-scan the hematoma showed localized highdensity area severely convex toward underlyingbrain.At recraniotomy, the hematoma consisted of massiveclot presented locally beneath the dural substitute.
