I．はじめに

　若年者に原発性橋出血をみることは非常に稀である．CTスキャン導入以前は，臨床的にこれを診断することは不可能に近く，剖検により診断されていたようである．CT導入以降は臨床的に診断できる可能性が高まったとはいえるが，この部にできた血腫は一般の脳内血腫と異なり長期にわたりhigh densityを示すことが少なからずあり，かつそのhigh density areaが増大したり縮小するという傾向がみられ，これにつれて臨床症状も軽快増悪を繰返すといわれる．そこでCT導入以後においても，手術による確認によらなければ橋部のgliomaをはじめ，他の腫瘍性病変あるいは動静脈奇形との鑑別は容易でないようである．われわれは，若年者で4ヵ月以上の長期にわたりCT　上high densityを示し，しかも経過中増大傾向を認め，手術により確認しえた小脳橋背部出血の1例を経験したので，文献的考察を加え報告する．

A 16-year old girl was admitted to our service withchiefcomplaints of abruptly occurred severe headache,vomitingand facial asymmetry. The patient had no episode ofdisturbance of her consciousness after the onset. Onadmission, she was alert and neurologicalexaminationrevealed left-sided pelipheral trigeminal, abducensandfacial palsy with moderate bilateral horizontalnystagmus inher both eyes. No cerebellar sign and no weaknesswerenoted on her limbs. Computed tomography revealed asmall high density area in the left dorsal part ofthe pons.
