脳腫瘍摘出術後に発生し，自然治癒した外傷性脳動脈瘤

Spontaneous Healing of Traumatic Aneurysm Occurring after Brain Tumor Removal

I．はじめに

　頭部外傷に続発する外傷性脳動脈瘤の報告が多数なされてきたが．近年，脳動脈瘤，脳腫瘍等，頭蓋内直達手術の機会が急増し，手術手技による動脈損傷に続発する外傷性脳動脈瘤の存在も無視できぬものとなっている^25）．特に，術後検査としてCT scanが脳血管写にとって代ることが多くなったため，その診断が遅れることもありうると思われる．

　今回，著者らは，脳腫瘍摘出術後，慢性期に紡錘状脳動脈瘤を脳血管写上認めた1例を経験した．経過を追跡したところ，発見8カ月後に動脈瘤は自然消失した．

A case of spontaneous healing of traumatic aneurysm occurring after brain tumor removal was reported.

A 29-year-old man was admitted to our hospital with complaints of dementia, gait disturbance and incontinence. During admission in the previous hospital, the diagnosis of oligodendroglioma in the right cingulate gyros was made and tumor removal was performed by LASER surgery. During vaporizing tumor tissue by LASER, there was a vigorous arterial bleeding which was controlled only by packing with spongel on the bleeding site.
