I．はじめに

　脳静脈・静脈洞血栓症については19世紀前半より種種の記載がある．初期には耳鼻の感染症に続発するものが知られ，のちには産褥期，経口避妊薬内服，血液疾患，手術の合併症として発症するもの，原因不明で特発性といわれるものが多数報告されている．しかも，その病態が障害される血行動態により，軽度の頭痛のみで経過するものから重篤なものまで多様で，診断が困難なことがあり，初期には剖検で確定診断がなされたことが多く致命率の高い疾患とされたが，近来では脳血管撮影で軽症例でも診断が可能となり，死亡率としては低下した．しかし，急性期に重篤な経過をとる症例に対する治療は，現在もなお議論のあるところである．

　著者らは頭痛，片麻痺，けいれんで発症し，頭蓋内圧亢進，出血性梗塞に対し減圧開頭術を行い救命しえた1例を経験し，その病態，検査および経過をふり返ってみたい．

A case of the cerebral venous and superior sagittal sinus thrombosis, treated successfully with the decompressive craniectomy, is presented here.

A 36 year-old man developed severe headache with nausea and vomiting. His consciousness was deteriorated gradually. Generalized or focal convulsion, right hemiplegia and aphasia were appeared.
