Abstract Purpose:To report a case of superior sagittal sinus thrombosis who showed blurred vision as the sole initial clinical sign and who resulted in irreversibly impaired visual acuity and visual field.

Case:A 43-year-old male was referred to us for visual field defect. He had noticed blurred vision 6 months before. Reportedly, both eyes showed edematous optic disc, with best-corrected visual acuity(BCVA)of 1.5 in either eye. Magnetic resonance imaging(MRI)of the brain showed no pathological findings.

Findings and Clinical Course:BCVA was 0.4 in the right eye and 0.8 in the left. Both eyes showed swelling of the optic disc. MRI showed enhanced short-TI inversion recovery around the optic nerve in both eyes. Pressure of cerebrospinal fluid was elevated at 42 cmH2O. Superior sagittal sinus thrombosis was suspected by magnetic resonance venography. The clinical features remained unchanged after anticoagulant treatment by heparin and intravascular thrombectomy. He then received ventriculoperitoneal shunt. Swelling of the optic disc decreased with final BCVA of 0.01 right and 1.2 left. Visual field showed defect in the inferior sector in the right eye and in the nasal-inferior sector with enlarged blind spot in the left eye. Headache was absent throughout.

Conclusion:This patient illustrates that thrombosis of cerebral venous sinus may manifest bilateral swelling of the optic disc as the sole clinical finding with no neurological symptoms including headache.
